The admission process at Toevlug Centre for Substance Use Disorders involves the following steps:

Complete the Application Form

The application form must be completed by a qualified professional, such as:

  • Social worker.
  • Psychologist.
  • Doctor.
  • Psychiatrist.
  • Pastoral counsellor.

  • Social auxiliary worker.

The application form includes a medical section, which must be completed by a medical doctor.  This ensures the applicant’s medical and psychiatric stability before admission.

Important Requirements

  • The medical report must not be older than 3 months.

  • Application forms can be downloaded from our website or requested directly from Toevlug.

Submitting the Application Form

Please forward the completed applications form to opname@toevlug.org.

Approval and Admission

After receiving the completed application form, our team will review and circulate it for approval.  Please note that incomplete application forms cannot be processed.  Once approved, the admissions officer will provide the patient with an admission date.
